Why campaigns underperform.

Almost nobody fails at advertising for interesting reasons. After enough audits, the same nine causes explain most underperforming wellness accounts.

The pattern

Most brands are running two or three causes at once.

Underperformance usually does not come from one mysterious platform issue. It comes from tracking gaps, wrong campaign setup, weak offers, landing page mismatch, slow follow-up or constant over-editing.

Account-level causes

The campaign may be optimising for the wrong thing.

Tracking is half-built

The pixel is installed but only fires PageView. Conversion tracking counts button clicks that never became bookings. Three different conversion actions can double-count the same lead.

The wrong objective or match type

Meta Traffic or Engagement objectives and Google Broad match with no negative keyword list both buy the least valuable audience each platform can find.

Budget spread too thin to learn

Five ad sets at ₹500 a day rarely learn. Two ad sets at ₹1,250 will beat five at ₹500 almost every time.

One creative concept, run to exhaustion

Frequency climbs, CTR falls and cost per lead rises. Meta needs genuinely different concepts, not four colour variants of the same image.

Offer-level causes

Sometimes the ad is carrying a weak offer.

The offer asks for nothing

Book a free consultation is not an offer. Specificity converts: a named assessment, fixed-price first session, limited slots or defined outcome.

The landing page does not match the ad

The ad promises a weight-management programme. The page is a general clinic homepage with nine services and a contact form in the footer.

Prohibited or weak health claims

Wellness advertising sits under stricter policy than most categories. Ads may be rejected, restricted or throttled quietly.

Operational causes

Some campaign problems are not ad problems.

The follow-up gap

Leads arrive at 8pm. Someone calls at noon the next day. By then the enquiry has booked with whoever answered first.

Impatience

Budgets changed every three days. Creative swapped weekly. Campaigns paused and restarted before they finish learning.

Constant intervention resets the learning phase repeatedly, so the algorithm never gets a stable run.

Match the symptom to the likely cause.

Lots of leads, no bookings

Wrong objective, or offer too easy to claim.

Cost per lead rising

Creative fatigue, frequency above 2.5.

Leads stopped suddenly

Ad rejection, policy flag, or budget exhausted.

Google spend high, conversions low

No negative keywords, Display Network left on.

Good CTR, poor conversion

Landing page mismatch or slow load.

Revenue is flat

Attribution double-counting, or no-shows.

The uncomfortable pattern

Not every campaign can be fixed inside the ad account.

Three of the nine causes are not marketing problems at all. They are offer problems and operations problems. A better agency, bigger budget or new creative will not fix a front desk, offer or booking process issue.

Any agency unwilling to tell you that is not auditing your account. They are selling you one.
